Login redirects to Let Us Know

I just registered an account and verified it by email, but when I try to login I keep getting redirected to the Let Us Know page (https://app.outagesio.com/site/letUsKnow). I fill in the three fields and click Send, but it just keeps bringing me back to the page to fill out the fields again. I also tried using different browsers and different operating systems but get the same result. Anyone else experience this?

It seems that avoiding the sign up questions sends the person back to them.
I just tested and it seems to work fine. We know from experience that even when we test something, there could still be a problem that we just aren't seeing. Can you continue with the sign up and see where you get and let us know.
About the sign up questions
Those questions are very important for us. They help us to understand how we can let people know about our service.
It is very difficult to compete for ad space against mega corporations that would prefer you not find services like ours that will help you to make them more accountable for what you pay for.

Thanks for the quick reply. I filled out the three fields and submitted, receive a message at the top that says "Thank you for your time" and the form appears again below it. So I filled it out again and submitted again and got the same message and empty form. Screenshots below:

I notice the form is cut off in your images. Are you using a mobile or is it just that the image isn't fitting for what ever reason?
To save you time, we are happy to help you with this. Can you provide some info that can help us to know what your account name is on OCP.
For example, is your email on OCP the same as the one you used to sign up into the forums?
If so, I can dig that up and find your account on OCP.

The form is cut off because I wanted to keep the images smaller for posting on the forum, so I selected the relevant portions of the screen.
Yes, my email on the forum is the same on OCP. If you're having trouble locating it I can DM you additional details. Thanks for your help!

I've sent this to the person that can find out what is going on. Strangely, we are seeing others signing up so definitely need developer input on this.
Unfortunately, it's a day where he is out most of the day but I'm sure he will reply as soon as he learns about this and we'll update here.

Hi again,
We spent time going over all of the logs, traces and things that could help us to see where this problem might be but we cannot find anything. In fact, others have signed up today, some answered, some did not and no one got stuck.
The dev says that if you are willing, a google meeting to see what is going on could certainly help because at this point, we cannot find a problem. If you are willing to do that, I can have a dev contact you directly.

Thanks again for your assistance. I tried a bunch of different combinations and I finally got it to work. It turns out my OCP profile had a missing last name. After I filled it in I passed the Let Us Know page and can now see the agent page. On the Update Account page, the last name field wasn't marked as required so I left it blank before, and I think that was causing the looping to occur.
